We provide support to people in their own homes and the community, to access opportunities in education, leisure and employment. We promote choice and respect to empower people in building life skills while maintaining positive health and emotional wellbeing.
How we help
We offer community based support to individuals either in their own homes or out in the community, we support people to develop independent living skills, achieve social inclusion and access community activities/groups/opportunities which they will then be able to independently. We do a lot work with health and wellbeing of individuals, supporting them to achieve a regular healthy lifestyle, accessing resources such as exercise on prescription and dieticians.
Who we can help
Our service is available to any individual being supported by the local community mental health trust. Alternatively people can set up private contracts and payments.
If you have a care co-ordinator then they can complete a referral form and we will arrange a time between all of us to do an assessment. If you don’t have a care co-ordinator you can ask to be referred to the community mental health team through your GP. If you’d like to arrange a privately funded support package we would come and meet you and discuss your support needs and complete a contract between us.
